Job Summary:

Job Responsibilities:

Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.

-Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.

-Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.

-Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.

Required Skills &

Experience:  One to two (1-2) years of experience.   

Preferred Skills & Experience: 

-Minimum of one (1) year of experience in home health care.

Required Education: 

-DPT from CAPTE accredited program. 

Preferred Education:  -N/A 

Required Certifications & Licensure: 

-Licensure as a physical therapist in the state of Florida. 

-BLS from the American Red Cross or American Heart Association.
